Your J2 can choose to continue their coverage during their vacation outside the US. So long as they are traveling to a country that is not their home country, the insurance would still cover them.
Some J visa sponsors will require them to keep their J visa insurance active for the entire time of their DS-2019 even if they leave the country.
If neither of those situations apply, they could let their coverage lapse and buy a new plan when they return to the US.

Maternity coverage for a J-2 visa holder is hard to find and very expensive. Also, it will only cover a pregnancy if you become pregnant after the plan is effective. If your wife is pregnant before the plan is effective, then I have no plans that will cover the maternity.
For the maternity, you can purchase IMG Student Health Advantage Platinum for you and your wife. You can purchase less expensive coverage for your 6 month old child. Most people find the maternity coverage for a J-2 visa holder to be too high, and choose not to have the maternity covered by insurance.
If you decide to purchase coverage without maternity here’s what I suggest:
The most economical way to purchase the insurance for your family of 3 is to buy two insurance plans separately:
The first application for one adult and the child (6 months old ) to buy the individual plan together. Since your child is under 10, you can get one child insurance free.
The second application for the other adult is the Taian Group plan with 10% off.

1. The purchase instruction for the first application:
One adult from your family (either you or your wife) will purchase with your child (6 months old). The adult is the primary insured in this plan and the child is the dependent. For each adult insured there is no cost for a dependent child under 10. Children free is only available on Individual plans, not group plans. Please click “ADD DEPENDENT(S)” while you’re filling out the form to add your child(ren) who is traveling with you. Please follow the instructions below to buy Plan B or Plan S. For this plan.

5) Select your deductible type. All deductible options meet USDOS standards. On the price chart, make sure you select a Maximum Limit of at least $100,000 in order to meet USDOS standards. For example, for Plan B with a $100,000 per illness/injury benefit and a $250 annual deductible (our most popular plan), the cost is $65.52/month for someone between the ages of 25 and 49. For Plan S with a $100,000 per illness/injury benefit and a $500 annual deductible (our least expensive option), the cost is $49.74/month for someone between the ages of 25 and 49.
6) For Fulfillment Method, you can select to receive fulfillment by either email or mail if you are in the US. If you are not in the U.S., please select to receive fulfillment via email.
After you have successfully submitted your application online, you will receive an Email from IMG in a few minutes. This email will contain your insurance ID card and your confirmation letter (Visa Letter). You can send the confirmation letter to your school as proof that you have acquired medical insurance that meets all their requirements.
2. The purchase instruction for the second application:
The rest of your family members (one adult) will purchase Taian Special group plan separately. Taian Special Group provides the same insurance coverage as individual plans with a 10% discount on premiums.
Plan B with $0 deductible is the plan with the best benefits.
Plan B with $250 deductible is the most popular plan.
Plan S with $500 deductible is the lowest price plan.

TaiAn Plan B with a $100,000 benefit per illness or injury and a $250 annual deductible is the most popular plan that meets these requirements. Plan S with a $100,000 benefit per illness or injury and a $500 annual deductible is the least expensive plan that meets these requirements. The biggest difference between Plan B and Plan S is that Plan B has no coinsurance (insurance will pay 100% of eligible expenses after the insured pays the deductible) while Plan S has 20% coinsurance in UnitedHealthcare Options Network or 30% coinsurance out of Network.

For this plan each person will register and pay separately. There is a $5 registration fee for each purchase or renewal. So it’s better to purchase the whole period instead of renewing every month.
For the most popular plan B-250, the total premium for 18 months for both ( one adult one 10 year old) will be: (58.97*18+5 )+(43.97*18+5)= 1,862.92.
For the cheapest plan S-500, the total premium for 18 months for both (one adult one 10 year old) will be (44.77*18+5)+(36.97*18+5)= 1,481.32
The Plan B is more expensive because it has a lower deductible, and no coinsurance. The lower deductible means the plan will start paying medical expenses after your first $250 of medical cost. The lower coinsurance means after the deductible the plan B will cover 100% of covered expenses, plan S will still leave you responsible for 25% of covered medical expenses.
